　　【摘要】 目的 总结我院使用微创闭合穿钉外固定架治疗肱骨骨折46例的体会。方法 全部病例均在DSA透视下进行闭合复位并结合我们改进的技术安置外固定支架固定，术后早期进行患肢功能锻炼。结果 有病例均无桡神经手术损伤并发症，在42例随访病例中，总优良率达95%，取得了满意的疗效。结论 该微创手术方法对治疗肱骨干中段、中下段各类型骨折，都是一种操作简单、安全适用、牢固有效的固定方法，避免了切开复位可能带来的医源性损伤和二次手术之苦。

　　Application of minimally invasive therapy with external fixator in the treatment of humeral shaft fracture

　
　　【Abstract】 Objective To summarize our experience in the applications of miniamlly invasive therapy suregery with close reduction and external fixator in the treatment of46patients with humeral shaft fracture.Methods All cases were treated with close reduction under the guidance of digital subtraction angiography（DSA）and fixed by external fixator according to our improved skill.They performed excises for their ill arm function at early stage.Results There was no complication of radial nerve injuries for all patients.Of42cases followed up，40were evaluated as exˉcellent（95%）.Conclusion Minimally invasive therapywith close reduction and external fixator is an easy，safety and efficient method for all kinds humeral shaft fractures.It also avioids iatrogenic injuries and the second operation.

　　肱骨干骨折是临床常见的一类损伤，约占全身骨折的5%。其治疗方法通常是手法复位外固定或手术复位内固定，手术方法有加压钢板、Ender、交锁髓内针、记忆合金抱骨器等，但无论采用何种治疗办法，如何既能解决骨折复位、固定和早期功能锻炼的问题，又能避免骨不连和医源性桡神经损伤的发生，是临床治疗的方向。1996年以来我们采用单边可调外固定架在DSA透视下微创闭合复位固定治疗肱骨骨折46例，取得满意的治疗效果，现报告如下。

　　1 资料与方法

　　1.1 一般资料

　　本组46例患者中男36例，女10例，年龄18～65岁，平均34岁。致伤原因:交通事故伤31例，摔伤9例，运动性伤4例，扭伤2例。左侧19例，右侧27例。开放性骨折7例。中段20例，中下段26例。横断型7例，斜型12例，螺旋型15例，粉碎型12例，骨折均有明显移位。合并有其它部位伤的有23例，其中合并多处骨折的18例。新鲜骨折44例，陈旧性骨折2例。手术时间在伤后1天～5周。术前有桡神经损伤麻痹症状者4例。

　　所用材料中30例采用张家港宏图骨科器械厂生产的于仲嘉式大号线形外固定架，16例采用山东潍坊三维骨科器械研究所生产的加强型三维外固定架。

　　1.2 手术方法

　　全部病例手术均采用臂丛麻醉。有创口的先行清创，一期闭合伤口，合并它处骨折的病人按照主次顺序进行相应固定。常规消毒铺单，手术在DSA透视下，先行骨折复位，复位后由助手维持体位，平行肱骨外侧医药学论文
